The following tables and video frames show ALL the appearances (including viewing timings) of the Video Phone in all 26 episodes of UFO shown in production order. The Video Phone had a few modifications made during production: these changes have been outlined in the tables and video frame text.

19:54, Cmdr. Straker is at his desk with a model of the B-142, with his left hand he is pressing one of the Central Selection Switches whilst in communication with Lt. Ford.
At 19:55 Cmdr. Straker releases the key, closing the communication with Lt. Ford.

The Video Phone is its usual position on Cmdr. Straker's desk, with a cable connected into the left connection slot. It is possible to see that there are no ventilation slots on either side of the fin at the top of the Video Phone.

The Video Phone is shown on Dr. Ashvale's (Dr. A. Brűnner) desk from a left hand viewpoint. The Video Phone is displayed a few times throughout this scene from differing angles.

A very nice - if partial - front view of the Video Phone with Lt. Gay Ellis in a video call with Cmdr. Straker. It is interesting to note that Cmdr. Straker uses the 'M' key (second from right) to close the video call.
NB: my 'guess' is that Ed Bishop was asked by the Director to use this key, as the shot was made over his right shoulder, and any other key would not have been visible. Also, it is worth mentioning that the video feed with Lt. Ellis closes immediately Cmdr. Straker releases the 'M' key.

Cmdr. Straker has just seated himself at his desk: at 31:07, Cmdr. Straker uses (out of view) one of the Central Selector Switches to tell Lt. Ford? to "Get onto Lt. Grant at the Zeta Tracking Station to expect Col. Freeman. Cmdr. Straker releases the Switch at 31:17.
At around 31:14 onwards, it is clear to see that there are NO ventilation slots on top of the Video Phone.
At 31:19, Cmdr. Straker uses (out of view) one of the Selector Switches: possibly to close all communications as stated a little earlier, "No calls."

At 35:37 it is just possible to see a connecting cable connected to the rear of the Video Phone.

As the Video Phone comes into view at 03:20, it is possible to see that it now has two rows of ventilation slots on top of the casing. However, there are no cables connected to the rear of the Video Phone.

Cmdr. Straker in at his desk, and there is a rear left view of the Video Phone. Again, the ventilation slots at the top are clearly visible.
At 15:14, Cmdr. Straker uses the Central Selection Switches to accept a call from Control. He presses a switch (unseen) and takes a videocall call from Col. Foster in his SHADO Mobile. The video is also shown on the wallscreen opposite Cmdr. Straker's desk.

As Col. Foster crosses Cmdr. Straker's office, the Video Phone comes into view. Again, the new ventilation slots at the top are clearly visible. The two connection slots at the right hand side of the base of the Video Phone are clearly visible, but there are no connecting cables.
NB: The right hand slot appears to be slightly larger (higher) than the left slot.

At 38:18, Cmdr. Straker's Video Phone buzzes, and Cmdr. Straker goes around his desk to accept the call by pressing one of the Central Selector Switches. Again, the ventilation slots are even more visible in this sequence. It is also possible to see that the back casing of the Video Phone is very slightly inset in the casing.

At 32.22, Cmdr. Straker uses one of the Centre Selection Switches to connect with Dr. Jackson.
